By Phone: You can also get your Transcript for US Tax Returns for the last (3) years by calling IRS at 1-800-829-1040

By Mail: Use Form 4506-T to request Transcript for US Tax Returns for the last three (3) years. You can also designate a third party like us to receive the information direct from IRS.

Where to file. Mail or fax Form 4506-T to the address below for the state you lived in, or the state your business was in, when that return was filed. There are two address charts: one for individual transcripts (Form 1040 series and Form W-2) and one for all other transcripts.

For Ohio residents mail/fax signed Form to: IRS RAIVS Team, Stop 6705-B41, Kansas City, MO 64999. Fax # 1-816-292-6102. For other states please review the chart on the form.

IRS has also placed tax Kiosk in various locations across US for folks who do not have Internet Access. Kiosk provides a quick, self-service way to get IRS forms. Kiosk machine uses touch screens in both English and Spanish. Kiosk is located in downtown Cincinnati, at Peck Federal Building, 550 Main Street, Cincinnati, Ohio 45202.
